COTTAGE CASE MANAGER

The Prince Albert Grand Council is now inviting applications for a Cottage Case Manager for the Child Care & Education Centre. The Prince Albert Grand Child Care & Education Centre provides integrated therapeutic childcare and specialized educational programming for First Nations ICFS/MSS Agencies. The facility is located on the Chief Joseph Custer Reserve in Prince Albert.  

Duties & Responsibilities:

Under the direction of CCEC, the Cottage Case Manager will be responsible for the development and implementation of the PAGC-CCEC intake and admissions process; the case management framework; and discharge planning for all children admitted to the CCEC. Specific responsibilities include:

  • Coordination and supervision for the CCEC intake and admission process
  • Responsible for coordinating & chairing placement committee meetings
  • Coordinate and chair service plan development meetings
  • Provide consultation to CCEC staff regarding CCEC case management framework
  • Make decisions in conjunction with the CCEC treatment team whether or not changes to the treatment approach and service plans are appropriate
  • Ensure all case management correspondences is forwarded to appropriate referring agencies as per CCEC designated time frames
  • Develop & implement quality assurance review format for CCEC case manager framework
  • Ensure children who are discharged from CCEC have a safe aftercare plan liaison between all programs with CCEC and outside agencies.

Education & Experience:

  • University degree in Social Work or other directly related field
  • At least 2 years direct applicable experience in therapeutic programming for children and youth
  • Understanding of First Nations culture, language and values in order to design services that respect these attributes
  • Ability to work well in a team approach to therapeutic programming for children
  • Excellent verbal and written communication skills
  • Group facilitation an asset
  • Ability to be a role model in a healthy lifestyle
  • Must have a valid Sask. Drivers License and the ability to travel

Must undergo a criminal record check, the results of which may influence the decision to employ.
